We have an open list and welcome requests for registration from patients living in or moving to the practice area. We accept patients in the SN5 and SN25 postcodes. You will need to be resident in the UK to qualify for NHS care**.
You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.
How to register with us
You will need to complete a registration form, a new patient health questionnaire, and signatures to confirm your registration request in person at the surgery. If you have a complex address or name we always recommend bringing an official printed version such as a driving license or passport as incorrectly spelled addresses or names will simply be rejected by the health authority and delay registration for you – sometimes by months.
If you have printed a form please bring this signed form duly completed with your current NHS no. We request that you bring identification and verification of address to register (important for matching records) but will not stop your registration IF all the forms are accurately filled in and we can match your record.
If you wish to get temporary care then you will be entitled to free emergency care.
**For non-emergency care patients ordinarily resident can ask for an appointment through reception and will be given depending on availability. For non-EU patients please check with our practice manager to see if you are eligible for NHS care before making an appointment. For non-EU patients who are not resident in the UK, there may be a charge for medical care – this is usually a standard charge for an appointment and additional charges for tests or prescription medicines.
